Danio 2013: Farne Islands Grounding

The cargo vessel Danio grounded off Longstone, Farne Islands, on 16 March 2013 with its chief officer asleep on watch and the BNWAS off (MAIB 8/2014).

The Danio grounding is a UK Marine Accident Investigation Branch case: the general cargo vessel ran aground at 0330 on 16 March 2013 at Longstone in the Farne Islands nature reserve, off the north-east coast of England. MAIB Accident Investigation Report 8/2014, published on 2 April 2014, found that the chief officer on watch had been asleep for more than 3 hours before the grounding, that no lookout was posted, and that the bridge navigational watch alarm system had been turned off.

The full article will cover the ship and its two-watchkeeper manning, the sequence of the grounding, the MAIB’s findings on fatigue and the circumvention of the BNWAS and the lookout, and the Chief Inspector’s statement that the barriers mandated against cumulative fatigue in short-sea shipping were not working. It will set the case against the MAIB’s 2004 Bridge Watchkeeping Safety Study and the later Lysblink Seaways and BBC Marmara groundings.
