The Dutch Safety Board, the Onderzoeksraad voor Veiligheid, is the independent body that investigates accidents in the Netherlands and on Netherlands-flagged ships. Its maritime investigations are safety investigations rather than proceedings to apportion blame, and it publishes its findings and recommendations openly.
